Directions:  The Teacher Candidate is responsible for initiating the completion of this document.  Both the Teacher Candidate and Mentor Teacher should review and sign this confidentiality agreement.  Do NOT turn a copy into UNCA.  The Teacher Candidate and Mentor Teacher should keep a copy for their records.

UNCA Teacher Candidate Confidentiality Agreement

Purpose:  For educational purposes and for meeting the needs of all students in the classroom during professional year, student teachers must access IEPs, assessment data and other pieces of identifiable information. The UNCA Teacher Candidate Confidentiality Agreement is intended to inform student teachers of their responsibility to protect personal and identifiable student information while performing tasks on behalf of their assigned school system.

In addition, it is critical for student teachers to engage in progress monitoring, making data driven decisions, the IEP process and IEP meetings.

  • Keep a copy for your records
