Directions:  The Teacher Candidate is responsible for initiating the completion of this document.  Both the Teacher Candidate and Supervising Teacher should review and sign this confidentiality agreement.  Do NOT turn a copy into UNCA.  The Teacher Candidate and Mentor Teacher should keep a copy for their records.

Purpose:  For educational purposes and for meeting the needs of students in the classroom during clinical practice, Teacher Candidates need to access data and other pieces of identifiable information.  This agreement is intended to inform Teacher Candidates about their responsibility to protect personal identifiable student information they need to handle while performing tasks on behalf of their assigned school system.

Steps for Completing Confidential Data Agreement:

  • Download agreement document
  • Read through the Confidential Data Access Agreement
  • Sign the agreement
  • Keep a copy for your records
